Most of the forms below are also available at Hope Church in the literature racks mounted on the wall opposite the door into the Church Office. For the "request" forms, there are two versions of each document - the content of the each form is identical, but how you can use the form differs slightly. We are making both versions of the form readily available to you so that you may choose to use whichever is most convenient for you.

How To Tutorial For Hope ChurchThis form is to be completed for building and grounds use by persons or groups beyond Hope Church's congregation and Hope congregant's requesting space for non-Hope Church functions.
Application For Use Of Building And Grounds - PDF versionThis form is for requesting the promotion of Hope Church activities, events, etc in our bulletin, weekly and monthly newsletters, website, projection system and outside marketing.
Event Form - PDF versionFundraising events are permitted for Hope Church's non-budgeted mission needs that enhance the Spiritual Growth of all persons involved. Fundraising events must be approved in advance by the Lead Team.
Fundraiser Application - PDF versionThis form is used to request the disbursement of budgeted funds for an approved purpose.
Purchase Fund Disbursement Requisition - PDF versionThis form is to be completed by persons or groups when requesting that an unbudgeted item be added to the Hope Church &Wish List& of donated items (an example might be a TV or DVD player for a classroom, or a volume of books for the library, etc). Wish List items must be approved in advance by the Trustees, and after the Trustees have approved the item, it will be listed here on the website (and elsewhere at Hope Church). The thought is that if a teacher or small group needs an unbudgeted item, if that need is publicized throughout Hope Church, someone may choose to donate the item to Hope Church for use by that class or small group.
Wishlist - PDF versionThis form is a combination of a covenant that the youth must sign, plus a parental consent form.
Youth Covenant Form and Medical and Permission Form - PDF versionA "PDF" file is generally a "read and print-only" document - you can read it and print it, but you must fill in the blanks by hand before bringing or mailing the completed form to the church office. PDF files generally print very nicely, and you do not need to buy any special software to open a PDF file (if you do not already have the free version of Adobe Reader on your computer, you can get it by clicking on the link at the bottom of this page). PDF files are generally not editable without advanced software, so remember if you choose the PDF version of a form, you will have to fill it out by hand.
A "DOC" version is a file that you can download and then complete on your computer using Microsoft Word (or a similar program if you do not Microsoft Word on your computer). After completing the DOC form, you can save it on your computer and then email it to the church office. Of course, if you prefer, you can also bring it in or mail it to the church office.
